Bilingual Case Manager -Gang Specialist

Under the supervision of the Director of Youth Development and using a wide variety of counseling modalities, the Gang Specialist exercises sound judgment in the treatment of children, youth, families, individuals and groups. Functions as part of a multi-disciplinary team, while making appropriate independent decisions for the benefit of the client(s). The Youth Development Gang Specialist will provide intensive one-to-one mentoring services for program youth and will develop curricula and facilitate prevention-focused psycho-educational groups for clients and their families. Provides expertise in areas such as sexual abuse, physical abuse, mental health, relational violence, substance abuse, physical health, education, Gang awareness and prevention, and community resources. Serves as a consultant in their areas of expertise and is available for home visits as necessary. Works in a culturally sensitive manner and abides by the legal and ethical standards of the profession.

Specific Activities
  • Develops and implements treatment plans.
  • Perform initial client screenings and assessments, including suitability and eligibility; provide program information; and conduct group and individual orientations.
  • Provides case conferencing in coordination with other professionals.
  • Provides crisis management consultation and education.
  • Mentors youth, develop curricula, and facilitate prevention-focused psycho-education groups for clients and their families related to current trends in Gang awareness and prevention.
  • Present clients with the harsh realities of gang involvement and its consequences.
  • Maintains current on all gang trends as it relates to the communities within Lifeline’s region of service.
  • Recruits targeted individuals for program participation, create and facilitate workshops, attend community meetings, surveys, and other activities by developing and distributing outreach materials, directly contacting targeted community members, networking with appropriate community organizations, schools, law enforcement and utilizing the broadcast and print media.
  • Develops, plan, coordinate and provide transportation to various outside functions and/or fieldtrips with clients and their families. 
  • Follows all legal and ethical guidelines pertaining to the treatment of youth and families as per state and county requirements (e.g., child abuse reporting, duty to warn, confidentiality, grievance procedures, serious incidents and elder abuse).
  • Participates in multi-disciplinary team meetings to develop appropriate interventions for youth and families, and serves as a consultant for the team in their various areas of expertise.
  • Participates in individual and group supervision both within Lifeline and within other organizations as needed.
  • Participates in quality assurance review and corrects any issues identified.
  • Develops and maintain positive professional relationships with community contacts and referrals including schools, law enforcement and San Diego County Juvenile Probation Department
  • Transports clients in a safe and secure manner in matters related to service delivery
  • Learn the pay structure required by the current contract and perform all necessary activities with clients and their families to ensure the accomplishment of the economic and programmatic success of the program.
  • Maintains files and records of individuals served, services provided, outreach activities conducted, surveys completed, and other general reporting as assigned.
  • Facilitates center-based prevention/positive development and treatment groups
  • Assists with program outreach.
  • Makes appropriate referrals as necessary.
  • Participates in wraparound treatment planning, implementation and collaborations.
  • Complies with all documentation requirements as stated in contract.
  • Provides all services in a culturally sensitive manner.
  • Participates in contract related and continuing education trainings.
  • Must demonstrate knowledge of trauma informed, client driven, and culturally competent care.
  • Completes all other related duties as assigned. 
     
Qualifications/Requirements
Holds a minimum of Bachelor’s Degree in counseling, social work, or related field, with training in child abuse and all other protective issues. Minimum one (1) year of experience working with youth in the area of violence prevention in education, law enforcement, counseling, or related setting. Experience must include counseling or intervening with “at risk” youth; presenting workshops, and/or training sessions in topics such as violence prevention, and gang intervention/prevention. Training in and mastery of family systems theory and application of diagnostic criteria from the DSM-5 and evidence-based practices. Bilingual English/Spanish required.
 
Must be able to make sound clinical decisions independently and be able to apply clinical judgment, ethics, and accountability to formulate and implement treatment plans and other clinical documentation. Can demonstrate ability to handle stressful and crisis situations tactfully and appropriately. Must demonstrate knowledge of trauma Informed, client driven, and culturally competent care.  Strictly adheres to the principles and practices of the counseling profession as elucidated by modalities, and to exercise discretion and independent judgment in all areas of job performance including adherence to appropriate professional boundaries.  Demonstrates strong leadership qualities, including but not limited to conflict management skills and inter and intra-agency collaborative skills.  Excellent written and verbal communication skills required. Must demonstrate interpersonal sensitivity and work with a diverse population.  Ability to establish and maintain effective work relationships. Must have reliable transportation, valid California Driver’s license, current automobile insurance. Hire is contingent upon passing pre-employment clearances, including criminal background checks, TB and Drug test.

    Pay Range Salary will commensurate with experience and education. Differential pay offered for bilingual skills.
 
 
Physical Demands/Work Environment
While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee may occasionally lift and/or move up to 20 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The employee may be exposed in a stressful environment where there are considerable mental and emotional demands.  Travel is required to multiple work sites and/or client homes using employee personal vehicle. Employee may be exposed to weather conditions prevalent at the time.  The employee will be required to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer, telephone, and fax machine.
