Bilingual Case Manager - Youth Development

Under the supervision of the FFT Clinical Program Manager, utilize a team approach to provide comprehensive case management, counseling and educational services to at-risk 12 to 17 year old youth and their families participating in the Family Functional Therapy (FFT) program. Services include, but are not limited to, in-home support, parent training, crisis intervention, counseling, case management, conducting placement assessments, transporting participating youth, and community education and development. The Case Manager uses a wide variety of modalities that exercise discretion and independent judgment in the treatment of children, youth, families, individuals and groups.  The professional is expected to function as part of a multi-disciplinary team while making sound, independent decisions for the benefit of the clients and their families.  The Case Manager works in a culturally sensitive manner and abides by the legal and ethical standards of the profession.

Specific Activities
  • Provide culturally competent assessments for youth and their families during office, school or home visits to identify client needs and challenges. 
  • Develop and implement strength-based individual and family case plans
  • Provide family focused psycho-education utilizing evidenced based strategies and a trauma informed approach
  • Provide crisis intervention and stabilization
  • Case conference in coordination with other mental health professionals including psychiatrists, psychologists, social workers and others
  • Provide in-home family support, case management and treatment (e.g. nutrition, budgeting, safety, home health and support systems), including developing family reunification plans
  • Adapt to flexible schedule which will may include working evenings and occasionally weekends
  • Advocate, mentor and role-model skills for families such as discipline, communication and general life skills
  • Conduct specific groups as necessary, e.g. parenting class, decision-making, substance abuse, anger management and life skills
  • Integrate restorative justice goals and activities into each client’s treatment plan as applicable to the offense(s) that led to their involvement in the justice system.
  • Work with clients on accepting accountability for their past behavior while supporting motivation and commitment to change.
  • Assist the youth and family to overcome challenging life circumstances that underlie or exacerbate the relational problems in the home through family support activities
  • Assist the employment-age youth and other family members with job readiness, job search, and guidance in obtaining apprenticeships/internships or on-the-job training that could increase their employability.
  • Collaborate with the family to address home health and safety issues
  • Consult and coordinate with other community providers to whom the youth has been referred, facilitate relationships between the youth’s school and outside service providers, and advocate on behalf of the youth and family with other entities as needed.
  • Become familiar with Family Functional Therapy model
  • Connect youth and families with supportive services that are more focused on remediating problems such as substance abuse, domestic violence, anger management, gang involvement, etc. that have the potential to interfere with the family’s progress if addressed sooner in the FFT process.
  • Help develop and maintain systems and procedures that encourage and support a multi-disciplinary service model that ensures timely and appropriate critical core services of:  information and outreach to the community, assessment, referral and follow-up, case coordination and management, home visiting, rapid response to crisis, drug and alcohol counseling.
  • Learn the pay structure required by the current contract and perform all necessary activities with clients and their families to ensure the accomplishment of the economic and programmatic success of the program.
  • Provide crisis management when needed.
  • Assist in career guidance, job skills and job placement for youth
  • Provide assistance with community resources through advocacy, information and direct connections
  • Ensure compliance with all recommendations from COR, the Juvenile Justice Coordinating Council, agency policies and procedures, and all laws of the federal, state, county and local governments.
  • Collaborate and maintain professional relationships with Juvenile Court, Juvenile Probation and Law Enforcement entities as specified by the FFT Program, to include receiving referrals and providing progress reports
  • Participate in all required meetings and trainings
  • Maintain necessary client records and statistics, and submit timely monthly reports
  • Participate in case supervision and program evaluation
  • Provide administrative duties and meet all contract requirements
  • Knowledge of and respect for diverse cultures
  • Other duties as assigned 
Qualifications/Requirements

B.A. or B.S. degree, in social work, psychology or related field, experience may be substituted. Bilingual REQUIRED (English/Spanish). Minimum 1-year experience in child, youth and family services field. Experience should preferably have been in a community based or neighborhood service organization. Experience in the areas of individual, group, and family counseling, outreach, community education and development, advocacy and case management. Ability to work with a diverse cultural population.

 Must be able to apply sound judgment, ethics and accountability to formulate and implement in depth assessment, service/case notes, case plans and other documentation. Demonstrates the ability to handle stressful and crisis situations tactfully and appropriately. Strictly adheres to professional principles and ethical practices, and exercises discretion and independent judgment in all areas of job performance including adherence to appropriate professional boundaries.  Knowledge and familiarity of juvenile justice system, gender specific programming for juvenile females and/or substance abuse assessment and counseling preferred. Demonstrates strong leadership qualities including, but not limited to, conflict management skills and inter- and intra-agency collaborative skills. Case Manager must possess excellent written and verbal communication skills.   Case Manager needs to have compatibility with agency philosophy and purpose, have knowledge of child development, family management, case management, delinquency prevention and crisis intervention issues, trauma informed approaches, and must possess excellent problem solving skills. Experience in Family Functional Therapy model preferred.
Some off-site services require own reliable transportation (mileage reimbursed), including a California Driver’s license, automobile insurance and must be eligible for approval to the agency’s approved drivers list.  Hire is contingent upon successful completion of pre-employment clearances including TB & Drug test and criminal background checks.

    Pay Range Salary will commensurate with experience and education. Differential pay offered for bilingual skills.

While performing the duties of this job, the employee is frequently required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee may occasionally lift and/or move up to 15 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The employee will be required to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer.

Work Schedule
40 hours a week- Monday – Friday- Flexible hours- may include evenings and weekends.
