Youth and Young Adult Liaison (YYA)

LifeSpring is a growing program that assists youth and young adults who are exiting the foster care system as well as youth and young adults who are experiencing homelessness.
Under the general supervision of the LifeSpring Program Manager, assures that all services provided to LifeSpring participants are being implemented through a youth driven, wellness and recovery model that is strength based, culturally relevant, trauma informed, and outcome based that supports self-sufficiency. The Youth and Young Adult (YYA) Liaison provides intensive case management and independent living skills development to the participants.  Assist youth in developing skills, obtaining employment, pursuing educational goals, and developing life plans that will support independence. Will conduct intakes, eligibility screenings, and basic needs assessments. In addition, will assist with housing navigation. Possess knowledge and expertise in relationship to transitional age youth, specifically former foster youth and homeless youth, and the specific issues surrounding this population and demonstrate excellent engagement skills. Provide skill development opportunities for youth including life skills training, meditation, economic literacy, transportation assistance, public benefits advocacy, financial assistance, and tenancy training. Provide supportive services to parenting youth including individualized housing, assistance securing childcare, education and support, maternity/paternity provisions, and specialized services for children. Provides linkage to services within the community to support transition to independence as well as aftercare, including but not limited to, mental health services, educational (college or post-secondary training) and employment support, etc. Provide crisis management and safety planning to the youth in the program. Assist youth and young adults in developing and maintaining permanent connections.  Participation in county collaborative meetings to identify service gaps, barriers, and successes in order to create best practices, policies, procedures, and protocols. Specific Activities
  • Utilize therapeutic engagement strategies to build rapport, identify strengths and barriers to participant self-sufficiency, and empower youth to achieve goals and permanent independence.
  • Provide intensive and therapeutic case management services designed to build and support participants’ relationships with family and community with an emphasis on permanent connections.
  • Utilize evidenced based practices to engage youth in participation of services that are client-centered, culturally appropriate, trauma-informed, in order to improve the short/long-term health, safety, and well-being of youth.
  • Utilize evidence based or promising practices as the preferred treatment modality for crisis management and safety planning, as well as individual and group sessions.
  • Provide coordination of care with other mental health professionals, primary care providers, and other supportive services.
  • Coordinate and engage youth in activities in the community to stimulate alternative interests, improve emotional and physical wellbeing, enhance self-esteem, and foster personal growth and satisfaction.
  • Provide support, advocacy, life skills, referrals, crisis intervention, and employment readiness services.
  • Carry a reasonable caseload required by funding source and provide responsive services in the office, home, and/or community setting.
  • Provide outreach and marketing to prospective participants and community partners.
  • Work closely with Housing Specialist to screen and interview prospective participants for the program.
  • Conduct a Youth Homeless Prevention Screening to determine participant’s level of prevention or diversion services priorities.
  • Will utilize the Coordinated Entry System to connect youth and young adults in finding safe, stable and inclusive housing options.
  • Will assist in the recruitment, hiring and training of lived experience experts.
  • Work with youth and young adults to explore the 3 E’s (Education, Employment, Entrepreneurialism) and assist in connecting them to the appropriate community partner so they can get support in the pursuance of their identified goals.
  • Assist youth in obtaining documentation, housing, education and legal services, etc.
  • Assess appropriate applicants and co-create an individualized case plan.
  • Monitor participant case plans and provide assistance when needed.
  • Provide youth with opportunities for leadership development.
  • Track short and long term client progress in Efforts to Outcomes (ETO) as required by contract.
  • Maintain accurate, timely documentation for each client.
  • Participate in 24 hour on-call crisis rotation twice monthly in 7 day increments.
  • Maintain linkages with community, educational, and employment resources.
  • Assist in program development and resource identification.
  • Maintain confidentiality and privacy standards of participants’ protected health information and other applicable information in accordance with HIPAA.
  • Maintain collaboration with County ILS workers, ILS contractors, social workers, probation officers, and other required parties in development and delivery of service plans.
  • Participate in multi-disciplinary teams, serving as an expert on matters related to behavioral health.
  • Attend and participate in program meetings, group supervision, retreats, committees, trainings, and other collaborative activities as necessary.
  • Uphold LifeSpring program compliance and contractual standards through direct monitoring of participants.
  • Ability to work independently and use good judgment.
  • Work in conjunction with other programs to create and uphold to monthly events in house or offered through the community.
  • Complete all other duties as assigned.
 
 
Qualifications/Requirements
BA in social work or related field + 2 years relevant experience required. Experience working with teens and/or transitional age youth who are experiencing homelessness or who are at risk of homelessness.
 
Strong skills in engagement, crisis intervention, case management, interviewing, rapport building, assessment, goal setting, and youth participation and development. Excellent verbal and written communication skills and computer literacy required. Must be highly organized, detail oriented, and self-motivated. Must possess working knowledge of trauma-informed care and dual diagnosis treatment. Must possess a strong understanding of mental/behavioral health. Must be able to work independently, use good judgment, and decision-making. Ability to work with minimal supervision and focus on multiple duties.  Available for alternating evening and weekend on-call shifts for crisis intervention. Belief in the purpose of North County Lifeline and our mission, vision, core values, and philosophy.  Available nights and some weekends for groups, events, and/or outings.
 
Other Requirements
Reliable transportation. Must have a California driver license and good driving record.   Willingness to use vehicle to attend meetings throughout San Diego County and transportation of youth to community activities and appointments (mileage reimbursement provided). Employment subject to clearances, including background finger printing checks, driving record, tuberculosis testing, and drug testing.

Salary commensurate with experience and education. Differential pay offered for bilingual skills.

 
 
Physical Demands/Work Environment
While performing the duties of this job, the employee is frequently required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee may occasionally lift and/or move up to 15 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
While performing the duties of this job, the employee may be exposed in an extremely stressful environment where there are considerable mental and emotional demands. Travel is required to attend meetings throughout San Diego County. Employee may be exposed to weather conditions prevalent at the time.
 
 
Work Schedule
Times may vary per individual client needs (may include evening and weekends). Position includes evening and weekend on-call shifts for crisis response and intervention.
